Care services

The people behind
the follow-through.

Add RN-supervised navigation to Amana’s software, or use the software with your own team. Care services are optional and scoped around your clinical care plan.

An extension of your care workflow

Keep the patient
connection going.

An Amana care-service engagement can include enrollment outreach, regular check-ins, appointment and referral coordination, practical barriers and documented follow-up. Your written scope defines the tasks, coverage and clinical handoffs.

Medical assistants can serve as care navigators within defined responsibilities and RN oversight. Services requiring additional professional qualifications are scoped with those roles in place.

Your clinicians retain the clinical relationship and decisions about diagnosis, prescribing and treatment.

Who does what

A defined path
from contact to care.

Each engagement establishes scope, coverage and the point at which a concern moves to a qualified clinician.

01 / CARE NAVIGATOR

Connect and coordinate.

Reach out, document the conversation, help with practical tasks and route concerns according to the agreed workflow.

02 / RN OVERSIGHT

Review and escalate.

Supervise navigation work, review concerns within nursing scope and follow the defined route to the treating team.

03 / TREATING CLINICIAN

Direct clinical care.

Own clinical assessment and treatment decisions, with the required specialist roles included for the selected model.

Getting started

A launch plan
built together.

Timing follows the program, staffing and system requirements. We agree on responsibilities before patient outreach begins.

01

Map the need.

Identify the patient population, care programs, clinical owners and tasks you want Amana to support.

02

Define the handoffs.

Agree on staffing locations, service hours, RN licensure for the states served, escalation routes, consent and patient communication.

03

Prepare the workflow.

Review data access, EHR needs, documentation, reporting and the patient introduction process.

04

Launch and review.

Begin the agreed work, review open items and use documented activity to refine the process.

Common questions

Clarity before
the first call.

Can we use our own care team?

Yes. Amana can provide the platform for your staff. If you need service support, we scope the responsibilities separately around your clinical model.

Does Amana replace our clinicians?

No. Navigators support the coordination and follow-through. Diagnosis, prescribing, treatment and other clinical services remain with appropriately qualified clinicians.

Which programs need specialist arrangements?

CoCM requires a confirmed collaborative-care team, including psychiatric consultation. MTM coordination connects patients to the plan’s qualified provider. Other programs also require the right personnel, supervision and workflow.

How long does launch take?

Timing depends on the program, staffing, system access and workflow requirements. We agree on a launch plan with your organization before patient outreach begins. Eligibility and payment depend on the applicable service and payer requirements.
